New Patient & Appointment Information:
- New patients are asked to arrive 15 minutes prior to their appointment to complete paperwork.
- Specific instructions will be given for certain appointments.
- Please give 24 hours notice if you need to cancel.
- Please bring past medical records and x-rays if you are transferring your care.
- Please bring your insurance card and picture identification.
- Copayments are due at time of service.
Referrals:
Referrals require an order by one of our physicians. The clinic will request the referral appointment and then communicate with you. We are not able to provide retroactive referrals.
Pharmacy Refills:
Please call your pharmacy to refill medications. The pharmacy will in turn contact the clinic. You should allow 24 hours for your refills to be completed.
