Medical Records
If you find that you need a copy of your medical record, we will be happy to provide it upon receipt of your signed release. You will need to show proof of identity.
Washington State law sets the fees charged for copies of medical records. There is, however, no charge for sending your records to your physician or health care provider or another facility.
If you do not need copies, but wish to view your record, you can call the Medical Records department at 360-435-0502 and schedule a time to do so.
Birth and death certificates are not held in CVH records. To obtain an official copy, contact the Snohomish County Health Department at 425-339-5280.
